Thats the only info you need to receive a new I20 from school to extend a opt.

Remember you can also still be in unpaid training while in OPT, so issue of payroll should not come. You dont send the docs directly to uscis. Have to send this info to school and they will then issue you a new i20 . You attach a copy of this i20 which asks for the extension along with other required docs. Dont worry too much.
